This page includes some basic information about our office hours, insurance and billing procedures, prescription refills and new patient registration. Please call the direct number at 603.431.1121 PRESCRIPTIONS Prescription refills:Prescription refills: Our Prescription Refill line is available for patient use 24 hours a day at 603.559.8329. These messages are picked up during appointment hours only and there is a 24-48 hour turn-around time. We will address your prescription refill as soon as possible. The 48 hour turnaround time takes weekends and holidays into account. Narcotic prescriptions must be picked up in person at our office by 4:30pm. A valid form of picture ID is required for all prescription pick-ups. NEW PATIENTS: Please click below to open a printable PDF version of our Patient Registration Form. You can fill it online, save and email it to us, or bring it with you to your appointment: When you check in for your appointment, you will be asked for a photo ID. Please understand this is for your personal protection. All minor children must be accompanied by a parent or legal guardian. If you are the legal guardian, you must bring with you paperwork proving legal guardianship to place in the child's chart.
